• For each intervention, describe 1 health improvement/outcome you hope to accomplish for each of the interventions. Total of 2 outcomes 5 points each for 10 points.
• Include how would you measure each outcome. 5 points each for a total of 10 points. Include 1 measurable outcome for each intervention for a total of 2. Be specific about how you would measure. For example; effectiveness of diabetic teaching could be measured by evaluating the A1C or the effectiveness of education about hypertension could be measured by BP.
