Engagement of stakeholder in an organization id critical in ensuring effective and efficient processes. Stakeholder are important part of an organization as they enhance the operational processes and the connection of a given facility to the external sources. There are different stakeholder agency reporting sources. Stakeholder groups may enhance collaboration and networking processes, specifically when they are able to include leaders and experts from different areas. Engaging the stakeholders in the continuous quality improvement enable an organization to have timely, relevant, as well as actionable outcomes thereby ensuring an initiative’s success (Bellucci & Manetti, 2018). Through fostering collaboration among the stakeholders, states as well as other organizations chasing quality in healthcare delivery can apply available skills and competency more effectively and efficiently. 

   One of the stakeholder agency reporting sources is the governmental institutions. In the process of continuous quality improvement, regulations from the government are critical as they inform the ideal processes that needs to be put into consideration in ensuring. Government is one of the critical stakeholder in the healthcare system (Maon, 2018). Another stakeholder agency reporting source is the research and funding agencies that are concerned with the operational processes of a given facility. These agencies may have a solution to the planned practices as well as the stipulation of the regulations that may enhance overall operational processes. 

The above stakeholder agency reporting sources may either contribute or hinder continuous quality improvement processes. Governmental agencies, as well as the funding groups, often contribute effectively towards the continuous quality improvement; they may stimulate regulations that can enhance the whole processes of quality improvement. In some cases, these agencies, through regulations, may hinder the processes put forward to promote continues quality improvement (World Health Organization, 2019).

Stakeholders are a person, group or organization that has interest or concern in an organization. In healthcare the main stakeholders are patients, providers, mayors and policymakers. “(Lubbeke, Carr, &Hoffmeyer, 2019)Patients are the single most important stakeholder group with regard to the learning healthcare system. They are both donors of personal clinical data and ultimate beneficiaries from the knowledge gained. 

One stakeholder reporting source is Centers for Disease Control. The CDC fights disease and supports communities and citizens to do the same. “The CDC conducts critical science and provides health information that protects our nation against expensive and dangerous health threats.” ( Centers for Disease Control, 2022) They base all public health decisions on the highest quality scientific data that is derived openly and objectively. The CDC is on the cutting edge of health security and they track diseases and find the most effective ways to prevent it. The second stakeholder reporting agency is the Agency for Healthcare Research and Quality. “The AHRQ mission is to produce evidence to make healthcare safer, higher quality, more accessible, equitable, and affordable, and to work within the U.S. Department of Health and Human Services and with other partners to make sure that the evidence is understood and used.” (Agency for Healthcare Research and Quality,2022) 

Both the CDC and AHRQ contribute to continuous quality improvement. They both research important health issues and find the best ways of treating diseases and illness.Engaging stakeholders in your quality improvement initiatives help, ensure that the initiatives are implemented effectively and helps to achieve intended outcomes.

Data visualization is increasingly important in the communication of quality improvement data. Effective use of data visualization with graphics, dashboards, and other tools requires an understanding of why this approach works and how to optimize its effect. The most important part of any data strategy is making sure the data can be used by the people who can benefit. In healthcare visualization can be taken further by adding a narrative, telling a story with the data and graphics rather than presenting a static representation (AHC MEDIA, 2019). Outcome measures reflect the impact of the health care service or intervention on the health status of patients. An outcome is the result of numerous factors, many beyond providers’ control. Risk-adjustment methods; mathematical models that correct for differing characteristics within a population, such as patient health status which can help account for these factors. However, the science of risk adjustment is still evolving (Agency for Healthcare Research and Quality, 2015). 

Quality improvement is characterized as the ongoing efforts of all healthcare professionals, patients and their families, researchers, payers, planners, and educators to improve patient outcomes, system performance, and professional growth (Banerjee et al., 2019). A hospital readmission occurs when a patient has an unplanned admission to a hospital within a specific time period of discharge from an earlier or initial hospital stay. Preventable readmissions have turned into a critical challenge for the healthcare system globally, and hospitals seek care strategies that reduce the readmission burden. Some countries have developed hospital readmission reduction policies, and in some cases, these policies impose financial penalties for hospitals with high readmission rates (Lahijanian, Alvarado, 2021). 

Decision models are needed to help hospitals identify care strategies that avoid financial penalties, yet maintain balance among quality of care, the cost of care, and the hospital’s readmission reduction goals. We develop a multi-condition care strategy model to help hospitals prioritize treatment plans and allocate resources. The stochastic programming model has probabilistic constraints to control the expected readmission probability for a set of patients (Lahijanian, Alvarado, 2021). The model determines which care strategies will be the most cost-effective and the extent to which resources should be allocated to those initiatives to reach the desired readmission reduction targets and maintain high quality of care. A sensitivity analysis was conducted to explore the value of the model for low- and high-performing hospitals and multiple health conditions. Model outputs are valuable to hospitals as they examine the expected cost of hitting its target and the expected improvement to its readmission rates (Lahijanian, Alvarado, 2021).
