Hours must be detailed per shift and confirmed by your preceptor before submitting your assessment. Papers submitted without detailed and confirmed hours submitted to the CAP will not be graded.

Organizational data, such as readmission rates, hospital-acquired infections, falls, medication errors, staff satisfaction, serious safety events, and patient experience can be used to prioritize time, resources, and finances. Healthcare organizations and government agencies use benchmark data to compare the quality of organizational services and report the status of patient safety. Professional nurses play a critical role in collecting, reporting, and monitoring data to enhance healthcare safety, improve quality of care, and ensure cost-effectiveness.

Your practicum provides you with a unique opportunity to practice this aspect of community nursing by identifying a process issue related to your particular site and exploring the issue. Each assessment will build on the next, culminating in a mock presentation.

o   What type of institution are your practicum hours taking place in?

o   Describe the patient population of your practicum site.

o   Estimate how many full-time healthcare staff are employed at your practicum site.

·        What kind of nursing or healthcare decisions do you encounter in your practicum site? Are you in a position to make or have input in any of those decisions at your site?

o   What are the primary patient health outcomes for your practicum site?

·        Identify your process issue as it relates to a patient diagnosis and patient outcomes.

o   Summarize the healthcare process problem or issue you’ll explore.

o   Provide context, data, or information that substantiates the presence of the problem and its significance.

·        Analyze your chosen process for impact on the organization and the patient client in terms of quality, safety, and cost.

o   What are the causes of the problem? What do you think is the root cause?

o   Provide evidence to support your claims.

·        Format: Format your paper using APA style. Refer to APA Style Paper Tutorial [DOCX] for guidance in writing and formatting your paper. Be sure to include the following:

o   A title page and reference page. An abstract is not required.

o   Appropriate section headings.

·        Length: Your paper should be approximately 4–6 pages in length, not including the title page and reference page.

·        Supporting evidence: Cite at least five sources of scholarly or professional evidence that support your central ideas. Resources should be no more than five years old, relevant to your topic. Provide in-text citations relevant to the issue and references in APA format.

·        Proofreading: Proofread your paper, before you submit it, to minimize errors that could distract readers and make it more difficult for them to focus on its substance.
