Scenario
You are the Process Safety Manager for a medium-sized petrochemical company in the UK.
One of your company’s planned projects for expansion is building an Oil Storage Depot similar to Buncefield Oil Storage Depot, UK.
Task
Produce a report for the director in charge of operations in response to his/her request below.
“Dear PSM,
I am emailing about the Oil Storage Depot project planned for expansion between 2022 and 2025.
We have not yet received full details, but it is evident the complex is similar to that of Buncefield Oil Storage Depot, UK. As you are aware, the depot suffered a major explosion and fire in December 2005. The explosion registered 2.4 on the Richter Scale, breaking windows up to 1 km away. The subsequent fire burned for several days, destroying most of the site. Over 40 people were injured, and there was significant damage to commercial and residential property, but fortunately, no fatalities. The cost of the incident in terms of the damage to property and loss of business is estimated to have been in the region of £1 bn. I would like you to provide me with a report setting out what is known about the explosion and lessons we can take into consideration for this new project.
I don’t understand the (UK) law concerning process safety, so I would be grateful if you could explain this in the report. Please include:
1. Process safety regulations in the UK with particular reference to Buncefield Oil Storage Depot. What would you consider our regulatory duties in the UK with specific regard to explosion and fire?
2. Analysis of the accident using the HAZOP and FMEA.
I am not interested in complex probability calculations. However, your HAZOP and FMEA analysis should identify and present significant causes and consequences as well as detailed proactive and reactive barrier analysis.
3. Systematic analysis of the accident based on the foundations of effective process safety programs.

A marked difference between your writing style, knowledge and skill level demonstrated in-class discussion, any test conditions and that demonstrated in a coursework assignment may result in you having to undertake a Viva Voce in order to prove the coursework assignment is entirely your own work.
