” As an x-ray technologist I utilize evidence-based practice (EBP) in my work to produce diagnostic images while maintaining patient safety. One of the main concepts of EBP is the consolidation of clinical experience and the best scientific evidence to make decisions for the patient’s care (Sackett, 1996). This process involves: formulating an answerable question, acquiring, appraising and aggregating evidence, applying the evidence and clinical experience to reach a decision and evaluating the outcome of the decision (Barends et al, 2015). There are many sources of evidence that I use to guide my practice, these include findings from academic journals, statistics from hospital databases and my professional experience.
One way I ensure I’m using EBP is by keeping up with the latest advances in medical imaging by reading the latest articles in some journals. Recently, I came across an article that discussed how a variation of one of the typical views done for a wrist x-ray could help identify fractures in the distal radius (Gasheva et al., 2020). While I was completing a routine wrist x-ray for an outpatient I noticed an irregularity in the distal radius. I asked the radiologist if it would be acceptable to perform this specialized view and they agreed it would be beneficial and it turned out to better visualize a fracture. Another source of evidence I use are the statistics from the hospital’s database about patient visits in our department. Comparing weekly data to the baseline before and throughout the pandemic is useful to gauge the workload being done and whether the data justifies additional staff coverage or movement within the department to make better use of resources. Upon completing the readings I gained a deeper appreciation for EBP because I realized that I was not explicitly taught how to pursue it properly during my program and training. Rather it was my curiosity for advancements in my field that led me to seek and apply new knowledge.
As a future healthcare manager it is important to maximize efficiency given the finite resources available. Using the principles of evidence-based management to guide decisions can reduce ineffective and costly decisions to the organization (Barends et al, 2015). I instinctively try to find areas to improve efficiency in my work. For example, initially we were obtaining portable chest radiographs for COVID+ patients by entering their room with the machine and having to clean the machine afterwards for infection control purposes, a process that normally took 5 minutes to complete took 15 minutes. I found a number of articles that looked at the possibility of taking portable x-rays through the glass door so that the machine stayed outside. These articles found there was no difference in image quality yet increased exam efficiency (Gange et al., 2020). I had a positive conversation with my supervisor and radiologist about using this evidence in our practice and they agreed to trial this protocol and evaluate the results. In a future management role I hope to find further improvements in my department.”
