Assignment Brief:
Each student is to research and submit a written assignment report of the following accident involving aircraft VH-NGA and the circumstances associated with the collision with terrain accident reported by the Australian Transport Safety Bureau (ATSB) in its Aviation Occurrence Investigation.
The ATSB report can be found on the course Canvas module or downloaded from

Following the format in Table 1, the report is to provide your analysis of what occurred and discuss salient gaps and deficiencies from a human factors perspective using Reason’s Swiss Cheese Model and any other 1 of the other following prescribed human factor models (SHELL model, SCHELL model, PEAR model, Dirty Dozen model, HFACS model). If appropriate, the paper should also indicate any resultant changes to aviation regulations and legislation.
The paper should be in your own words as much as possible; where you need to quote from other sources you must properly acknowledge and reference those sources.
Your report must contain the following sections and heading descriptions as in Table 1.

Section 1: Table of Contents
A Table of Contents is expected for the report. Penalties may apply if the Table of Contents is not available, anomalies and/or inaccuracies are found.
Section 2: Table of Figures / Tables
A Table of Figures / Tables is expected for the report along with the report. Penalties may apply if there is no Table of Figures / Tables, anomalies and/or inaccuracies are found.
Section 3: Introduction and Background of the Accident
A description of the accident based on factual information should be provided in this section. Section 4: Literature Review
Present a critical evaluation and summary of published literature (from academic sources, peer-reviewed journal papers or articles/reports from authoritative industry stakeholders such as government agencies, ICAO) to establish current knowledge of human factors, safety and risk management issues against the safety issues specifically highlighted in the ATSB report for this accident.
The literature review should contain at least 2 text books plus an additional 2 technical literature issued by main aviation stakeholders, peer-reviewed journal papers or articles/reports from authoritative industry stakeholders.
A literature review is not just about reporting published facts; it requires careful consideration of the published literature to construct an unbiased narrative supported by published evidence.
Section 5: Analysis
You are to use the James Reason Swiss Cheese model plus any one other of the following human factor models (SHELL, SCHELL, PEAR, Dirty Dozen, HFACS) to analyse the cause of the accident.
Your analysis using the Swiss Cheese model and the other selected human factors model must describe and clearly identify the relationship of human factors issues contributing to the cause of the accident and the safety issues found by the ATSB report.’
Section 6: Active Failures and latent Condition Failures
List and describe any 6 failures (active failures or latent condition failures) that contributed to or led to the accident.
Section 7: Corrective and Preventive Actions
What are your recommendations to effectively correct and prevent each of the 6 listed failures in section 6 from re-occurring.
Section 8: Conclusion
The function of your paper’s conclusion is to restate the main argument. It reminds the reader of the strengths of your main argument(s) and reiterates the most important evidence supporting those argument(s).
Assessment Criteria:
To provide a comprehensive answer to the assessment brief, you will need to achieve the following, as a minimum:
•Display a comprehensive understanding of the topic;
•Demonstrate a wide review of the literature and/or include practical examples to support your argumentation and analysis (a minimum of two text books and/or technical literature issued by main aviation stakeholders, 2 journal peer-reviewed papers and 2 articles/reports from authoritative industry stakeholders are expected);
•Demonstrate a critical analysis of the literature and research, including your own arguments based on the facts and circumstances of the incident/accident.
•Present information in a logical, systematic and integrated manner (i.e., structure/format and written quality of the report);
Assignment Report Format:
The assignment report is to be submitted as a Microsoft Word Document only and it should be formatted on A4 size page layout, single-spaced, in a 12-point font such as Times New Roman, Courier or Palatino.
You should use appropriate graphs, diagrams and figures to augment the text – these should be numbered consecutively and be referred to in the text of your paper.
The full detail of the source(s) of the figure/graphs/diagrams also needs to be included in the assignment reference list.
A coversheet with the title, names, student numbers, date and word counting should be included.
Please write your text in good English. A high standard of presentation is expected and poorly presented work may have a significant impact on the final mark.
