A population health approach is usually a useful strategy that can help aid in the improvement of patient outcomes for some of the groups. Population health focus strategies can also be used in saving resources and in reducing the utilization of healthcare by minimizing the need for hospitalization and emergency department visits (Shahzad et al., 2019). One of the ways that population health focus can be used in transforming healthcare delivery is through segmenting patient populations. Some of the ways that populations can be grouped include age, disease, gender, and social factors (Farmanova, Baker, & Cohen, 2019). In most cases, risk stratification is used in addition to age and disease condition to provide a more precise view of the population that is being focused on. The use of risk stratification in patient segmentation is usually one of the first steps in crafting the right population health strategies. For instance, those at high risk are provided with care management while those with an increasing risk receive intervention care (Farmanova, Baker, & Cohen, 2019).
Population health focus can also lead to a transformation in the healthcare delivery process through utilization of primary medical care home delivery models (Farmanova, Baker, & Cohen, 2019). The foundation is usually the relationship that is there between the patient and the physician (Shahzad et al., 2019). The approach to population health helps in the promotion of wellness that aids in preventing illness from taking place. In such a case, patients can record reduced hospitalizations in addition to needing emergency care, which is integral in reducing the cost of care and promoting the attainment of better health outcomes for the patients.
Population health focus can also transform the healthcare delivery system through the use of evidence-based screening as well as prevention. It has been proved that only focusing on sick patients and those at a higher risk is not effective in controlling the cost of healthcare in the long-run (Shahzad et al., 2019). Preventing illness in a group of patients who may be at a higher risk if becoming sick is crucial in reducing the cost of healthcare.
Focusing on population health strategies as opposed to treatment of patients who are ill is considered important (Farmanova, Baker, & Cohen, 2019). The focus should always be on the creation of healthier communities through making investments that direct the healthcare system towards focusing on overall health as opposed to only focusing on the sick patients in the society (Shahzad et al., 2019). Such a move would help the healthcare system shift from the current volume-based approach in utilization to care that is value based.
