Purpose
This Focused Community Clinical Experience (FCCE) clinical learning assignment aims to prepare students for engagement and patient care, develop students' understanding of social determinants of health (SDOH), healthcare disparities, health equity, structural racism, and the impact on the community population. The priority nursing action plan will heighten students’ awareness of nurses’ role in advancing health equity for all and mitigating disparities.
Program outcomes: This activity enables the student to meet the following course outcomes. For complete alignment with associated outcomes and competencies, please refer to your Course Specific Clinical Learning Competency Alignment Table.
1. Provides individualized comprehensive care based on theories and principles of nursing and related disciplines to individuals, families, aggregates and communities, from entry to the healthcare system through long-term planning.
2. Demonstrates leadership and collaboration with consumers and other healthcare providers in providing care and/or delegating responsibilities for health promotion, illness prevention, health restoration, health maintenance and rehabilitative activities.
3. Communicates effectively with patient populations and other healthcare providers in managing the healthcare of individuals, families, aggregates and communities.
4. Integrates clinical judgment in professional decision making and implementation of the nursing process.
5. Demonstrates responsibility for continued personal and professional development through enrollment in graduate education, continuing education degree programs, professional reading and participation in professional organizations and community service.
6. Implements professional nursing standards by practicing within the legal definitions of nursing practice and acts in accordance with the nursing code of ethics and American Nurses Association (ANA) standards of practice.
7. Practices in established professional roles consistent with entry-level BSN graduates to provide cost-effective, quality healthcare to consumers in structured and unstructured settings.
8. Incorporates evidence-based practice in the provision of professional nursing care to individuals, families, aggregates and communities.
Assignment Requirements:
· Required training completed before the first scheduled FCCE clinical day (see below).
· Complete a minimum of four (4) hours of FCCE at a clinical site in each course with a clinical credit hour component.
· Complete and submit written assignment. The written assignment has two components: CORE (all students except those in NR441/NR442/NR444 complete) and Leveled (all students complete). The CORE assignment requires the submission of a priority action plan by each student. The Leveled assignment also requires a submission by each student. Follow the grading rubric. All assignments submitted must be typed and in APA format.
Required Training: The training listed below must be completed once, in NR326, NR327, NR328, NR341, NR342, NR442/NR444 depending on which is your first course participating in the FCCE. Students who (1) do not complete the training, (2) do not upload the certificate of completion, and (3) do not bring a hard copy of the completion certificate to the FCCE clinical site are not permitted to attend, resulting in an absence. Refer to the Student Handbook Attendance section for more details.
· Students must complete the following:
· CUL215 Addressing Implicit Bias: A Primer for Healthcare Providers (2 hours)
· Upload the certificate of completion to Week 8: Clinical Learning Assignment
· Show the certificate to the faculty/non faculty facilitator who is facilitating the clinical experience on the scheduled day
Due date: Your faculty member will inform you when the FCCE Core and Leveled (as appropriate) assignment is due. The assignment is uploaded to Week 8: Clinical Learning Assignment. The CUL215: Implicit Bias Training certificate is due before your scheduled FCCE and is uploaded to Week 8: Clinical Learning Assignment.
Grade possible: Satisfactory/Unsatisfactory. A Satisfactory rating meets the following on the Clinical Learning Evaluation:
· Clinical Learning Competency: Completes all clinical learning experiences and requirements successfully (PO 5).
· Performance Descriptor: Completes all assignments related to the clinical learning experience within established guidelines.
Preparing the assignment
Follow these guidelines when completing this assignment.
1. All students:
a. Complete the required training (see above).
b. Complete Part A: CORE assignment as outlined below (except students in NR441/NR442/NR444).
c. Complete Part B: Leveled assignment depending on which course the assignment is completed.
d. Be prepared to share and engage in dialogue during pre & post-conference. Consult with your faculty member if you have questions.
Part A: CORE Assignment (Pre-work, During focused community clinical experience FCCE day, and Post Focused Community Clinical Experience FCCE):
(ALL students EXCEPT for students in NR441/NR442/NR444 complete the CORE assignment):
1. Pre-work (prior to the community clinical experience day): In collaboration with your clinical group or independently, identify contributing social determinants of health (SDOH)/healthcare disparities, health equity factors, and structural racism in the community where your community agency is located and complete the following. Be prepared to engage in dialogue during preconference about your findings:
a. Explore the agency:
· What is their county and/or zip code?
· What is their vision/mission/purpose?
· Who do they serve?
· What services/resources do they offer?
b. Review video County Health Rankings & Roadmaps – Our Model (approximately 1 min).
c. Review county health rankings to understand the community your assigned agency resides in, exploring all areas of the rankings.
· Within the county health rankings website, type in the county or zip code and click search.
· Scroll down and review the information on the page.
· Identify three (3) Health Factors/Health Behaviors with the highest percentage, number, or ratio that could contribute to healthcare disparities and health equity opportunities.
· Review video County Health Rankings & Roadmaps – Our Model (approximately 1 min).
d. Identify the target aggregate population.
· Include specific populations as appropriate (i.e., Unsheltered/unhoused, LGBTQIA+, veterans, people with disabilities)

2. During focused community clinical experience (FCCE) day:
a. Pre-conference:
· Engage in dialogue with clinical faculty/non-faculty facilitator (preceptor/affiliate clinical nurse, etc.) and clinical group the findings from all aspects of the county health rankings compared to the national and global benchmarks.
· Ensure you note which has the highest percentages/number/ratio of factors related to Health Factors/Health Behaviors, Clinical Care, Social Economic Factors, and Physical Environment
· Include information gleaned from the interactive video and your review of the articles related to human trafficking and the unhoused.
· Use this information to collaborate with clinical faculty/preceptor/affiliate clinical nurse and the clinical group to identify SDOH/healthcare disparities, health equity factors, and structural racism in the community.
b. Clinical:
· Provide care to the population and ensure the use of Preferred Terms: Preferred Terms for Select Population Groups & Communities | Gateway to Health Communication | CDC .
· Evaluate for healthcare disparities contributing to health equity opportunities.
· Complete the Social Determinants of Health Questionnaire (SDOH).
· Engage agency leaders in dialogue about gender-expansive care, policies, or procedures.
· Explore what interprofessional collaboration opportunities exist in the agency to advance health equity.
c. Post-conference, in collaboration with your clinical group, discuss:
