File Note 11th January 2025:
The early years assistant in room 2 raised a concern regarding Sophies general health and welfare. It was noted that Sophie attended the creche wearing the same clothes 3 times this week despite the clothes having been stained during lunch time the first day they were worn. Sophies hair seems unkept and she is itching her head regularly showing some signs of discomfort.
The Creche Director and Room Leader in charge of Sophies room have acknowledged the staff members concern and agreed to review this situation over the next 2 days and meet at 4pm each day to follow up and decide if and what further action is required.

Sophie did not attend the creche today. When contacted Sophie’s mother advised she was unwell and would be attending tomorrow.
File Note 13th January 2025:
Sophie attended the creche today and while she was wearing a different t-shirt it was noted her trousers were the same as she had previously been wearing and that there were still soiled with no evidence they had been washed.
