Carry out the plan of care or support within the limits of own role
Lead others to carry out their responsibilities when implementing a plan of care or support
Adjust the plan in response to changing needs or circumstances

Agree on methods for monitoring the way a care plan is delivered
Collate and record monitoring information from agreed sources and agree on any changes
Seek agreement with the individual and others about who should be involved in the review process and the criteria to judge the effectiveness of the care plan
Seek feedback from the individual and others about whether the objectives of the plan have been met
Work with the individual and others to agree on any revisions to the plan
