Provide detailed evidence that the problem exists. This can be found in the literature or national data-bases (evidence-based approach), and/or you can include agency or unit data related to the identified problem. Clearly state the impact of the problem as a declarative sentence. Your problem statement should address the following: What is the impact of the issue? What is the impact on the stakeholders (i.e., patients/nurses/the organization)? Why is it important for this problem to be solved? What will happen if the problem is not solved? What attempts have been made to solve the problem? Write a conclusion that summarizes the information presented.
