After you help the same 84-year-old client (from DQ 1) ambulate to the bedside commode, you begin to put together a plan of care for her. While helping her, you noticed that she does not have good balance and she seems to be in pain. When you checked her surgical site there was no drainage on the dressing and it was intact.
In your post, state one priority nursing diagnosis for the client based on your assessment. Then provide a rationale for the nursing diagnosis of your choice.
Post three different responses to classmates with different diagnoses from yours and identify one nursing intervention and rationale for the different diagnoses.
