Assessment Description

A common measurement used to test central visual acuity is the Snellen Alphabet Chart. Describe the process by which the client’s vision is tested. How are the results documented? Provide an example and explain what it means. What should the nurse do if the client cannot read the largest letters on the chart?

Post three different responses to classmates with some additional suggestions on what the nurse can do for the client who cannot read the largest letters.

Discussion posts should be at least 200 words and responses should be between 100-150 words.
